Medicis Aesthetics and Restylane ® are pleased to offer an exciting new patient rewards program.
Now you can earn rewards for being a loyal Restylane customer. Simply begin your beauty regimen, and follow up with treatments every six months, or sooner. After your first treatment and each of the next four follow up treatments, you'll be rewarded with Restylane Rewards Benefit Checks of increasing value. The rewards grow over time and include a special bonus after your fifth treatment — a $200 treatment voucher towards your next Restylane treatment.
To participate, you'll need to submit a copy of your physician's invoice, plus the end flap from the Restylane box(es) that were used in your treatment. You'll also need to register at www.restylaneusa.com and then fill out and print a Restylane Rewards Redemption Form.
